DATSS aim to provide quality support to participants who receive NDIS funding. We provide experienced staff members to each participant and offer a range of different services and skill sets.

DIGNITY AND TRANSITION SUPPORT SERVICES PTY LTD is registered to provide supports throughout Australia as an approved registered NDIS provider.

  • Agency managed participants.  
  • Plan managed participants.
  • Self-managed participants.
  • Private participants Services offered including community participation, domestic assistance, personal care, wound care, social support.
  • Short Term Accommodation.
  • NDIS Participants who require Supported Independent Living (SIL).

Support Coordination

Our Support Coordinators Connect you with Support Services Across Southeast QLD. DATSS are currently providing support coordination. DATSS assists people with a disability to optimise their NDIS plan to achieve their goals. 

Our Support Coordinators will walk beside you to help build your confidence and capacity to understand and implement the funded supports in your plan. DATSS support Coordinators will assist with linking you to appropriate services to maximise your supports and use of your current plan. 

Child Safety

DATSS offers Essential Support services to young people under the age of 18 who currently receive care under the guidance of Child Safety.

DATSS works closely with Child Safety to ensure that each young person’s wellbeing and safety are met. Part of our core mission in this space is to offer resources and tailored supports to ensure there needs are met.

At DATSS we understand the unique needs of children and the importance of creating safe spaces for them to learn, grow and thrive for a successful future.

Our services are tailored to suit the needs of all children to provide a safe and educated future.

We offer a range of services in support of our young people:

  • Child Safety Education – Teaching children about personal safety, learning how to recognise unsafe situations and understanding how to seek help if needed.
  • Supervised activities- Organising safe, supervised events and activities that promote positive social interaction and development. This include assisting the young person in supported visits with family members or within a community setting whilst maintaining Privacy and Confidentiality.
  • Assistance with Education- Tailor made supports to encourage and promote education throughout the young persons school life.
  • Tailored Supports- Working along side Child Safety and Families to assist with respite needs or Residential homes in a 24/7 capacity.
  • At DATSS we believe that a proactive approach to safety is the best way to empower children, parents and communities. Our dedicated team is committed to providing the resources, education, and support necessary to create a safer world for our children to grown in.

Participant Referral Form

  1. This form must be completed so the appropriate coordinator can process and ensure the Participants receive the most timely and effective service to meet their needs while increasing their knowledge of community resources.
  2. Coordinator will then match the staff skill set against this referral form to ensure support staff are confident in their ability to deliver a quality service to participants, knowing the limitations to the capacity of their own service.
  3. An appointment will be scheduled to discuss capacity and placement options once the above steps have been finalised.
